1. MOBILE VETERINARY SURGERY APPOINTMENT

Thank you for expressing your interest in requesting a mobile Veterinary Surgery appointment. To secure a surgery appointment, kindly adhere to the following steps:

​

  1. Click on the "Request to Book" button, select a date and time, fill out your details, and click again on the "Request to Book" button.

  2. After submitting your "Request to Book" form, please check your email for a link to pay a $77 deposit fee. Note: If you cannot pay via credit/debit card, you can transfer the fee via Zelle to castilloanimalvc@gmail.com or send a check to our address at Sepulveda Blvd. #117 Mission Hills, CA 91345. Your appointment will not be secure until we receive your deposit fee. Please note that this deposit fee is reimbursable unless you cancel your surgery appointment less than 24 hours before the scheduled date.

  3. Fill out the Anesthesia/Surgery/Blood Work Consent Form.

  4. Fill out the Surgery Patient Registration Form, one per companion animal.

Please be aware that requesting an appointment on a specific day does not guarantee immediate availability for services in your area on that day. Upon requesting an appointment, you will receive an email with alternative available dates.

2. NEW OPTION - Sylmar/San Fernando Station: (On Frank Modugno Dr, between N Hubbard Ave and Village Way Dr)

Alternatively, you can visit our Sylmar/San Fernando Station location for surgery services on an arrival basis.  First-come, first-served. (Please look for the green and blue 26ft truck.)

Please note that for services at this location, customers should bring the Surgery Application Form,  the Anesthesia/Surgery/Blood Work Consent Formand the Surgery Patient Registration Form, printed, filled, and signed completely. Doing so will give you an advantage in the line. Click on each document to access a printable form, or printed forms are also available.

 

Please be aware that for services at all surgery locations, the following requirements must be met:

  • Cats must be inside a hard or sturdy labeled carrier with a blanket.

  • Feral cats should be inside traps and must get an ear tip.

  • Dogs must be leashed.

  • Absolutely no prong collars are accepted (recommended).​

  • All animals must be fasted for a minimum of 8 hours before their appointment, and kittens/puppies need to be at least 2 lbs of body weight.

  • 3. HOUSE CALL APPOINTMENT (Only Dr. Castillo comes to your home, no surgery services)

  •  Please note that to approve your request for a house call appointment, Castillo Animal Veterinary requires you to read, fill out, and sign the following two forms:
